What Information Can Be Found in Health Records?

Health records or vital records are maintained at both the state and county levels. They generally include birth and death records, marriage and divorce records, and adoption records. Vaccination records are also sometimes maintained by the health department, although not as frequently as the individual or parents of a minor are considered to be the primary party responsible for these records.
The type of information that you can find in health records includes:

Birth Certificates

  • Mother's and Father's name & occupation
  • Delivering physician's name
  • Date and place of birth

Certificates of Adoption

  • Child's name, date and place of birth
  • Place of adoption
  • Adoptive parent's names and occupations
  • Natural (birth) mother's name

Death Certificates

  • Date of death

How to Conduct a Health Records Search?

Health records or vital statistics are generally maintained at either the state or county level. Searches for these records may be available online, or requests can be made in writing through the pertinent office. Copies of records will typically only be given to the person named in the record or to kin of such person. To obtain access to such records, contact:

  • The relevant department will typically be called 'health', 'vital records', or something similar. These departments may only retain the most recent records, for example for events occurring over the past year.
  • The County Recorder's Office where the event took place. Older records will frequently be archived in the recorder's office where the event took place.
  • Doctors' offices or sometimes the relevant state health department's immunization program may retain records on immunizations.
